Acknowledgement of Risk and/or Release forms are required for all climbers. The Acknowledgement of Risk form is for both adults and minors climbing in Grand Teton National park. The Release or the Release for Minors form is required when activities outside of Grand Teton National Park are planned (e.g., Teton Village, Wind Rivers).

If you are obtaining this form for minor climbers to send to us in advance of planned activities, please complete both the Acknowledgement of Risk form and the Release for Minor form unless you are absolutely sure of where your child will be climbing. Please call the office if you have any questions.

For all Exum climbers with activities within Grand Teton National Park, including minors. If for a minor, the form must be signed by a parent or legal guardian (this excludes other relatives and caregivers). The form can be signed in advance and faxed to us at (307) 733-9613.
