MINORS

If you are obtaining this form for minor climbers to send to us in advance of planned activities, please complete the Acknowledgement of Risk form below. If possible, we also request that you complete the Registration Form. Please call the office at 307-733-2297 if you have any questions.

Acknowledgement of Risk
For all climbers with activities with Exum Mountain Guides, including minors. If for a minor, the form must be signed by a parent or legal guardian (this excludes other relatives and caregivers). The form can be signed in advance, scanned, and e-mailed to us at exum@exumguides.com. Please include the date(s) of the climbing activities in your e-mail, and the name(s) of other party members if applicable.
